How we collect consent

Before you can send the eligibility form, you must check a box agreeing to receive SMS and text messages from Vantic. The box is not pre-checked. You cannot submit without checking it. This is the opt-in as it appears on the form:

Where should we send your full diagnosis?

1. Who is sending the messages

Messages are sent by Vantic. For questions about these terms, contact us at info@vantic.ca.

2. What you agree to

By checking the box, you agree that Vantic may send you SMS and text messages about your request. That includes appointment confirmations and reminders, follow-up about your eligibility form or booking, and related operational messages (for example a payment or scheduling link when relevant).

3. Message frequency

Message frequency varies. You may receive a few messages around a form submission or booking, then occasional follow-up if you have an open request with us. We do not send unrelated promotional blasts.

4. Rates

Message and data rates may apply, depending on your mobile carrier and plan. Vantic does not charge a separate fee for SMS.

5. How to opt out

Reply STOP to any Vantic text to unsubscribe. You may also email info@vantic.ca and ask us to stop texting you. After you opt out, we will not send further SMS except a one-time confirmation that you were unsubscribed, unless you opt in again.

6. Help

Reply HELP to any Vantic text for help, or email info@vantic.ca.

7. Privacy

We use your phone number to send the messages you consented to, and to operate our services. We do not sell your phone number. See our Privacy Policy for how we collect, use, and retain personal information, including sharing with Twilio to deliver SMS.
